Phelps and the United States won this event back at the 2008 Summer Olympics in Beijing, with Australia taking the silver medal in that race and Japan settling for bronze on the day. The Americans also won this race at the 2011 FINA World Championships, finishing ahead of Australia and Germany at that event.

Heats for the men's 4x100m medley relay go on Friday, with the final set for Saturday and the United States pegged as the big 4/9 favorite on the Olympic Swimming Las Vegas Odds at Bovada Sportsbook.

Yang won this race at the 2011 FINA World Championships, with Cochrane ending up second, and Gergo Kis of Hungary (20/1) third.
